Avoiding and Managing Tube Exposure in Glaucoma Drainage Device Surgery
Presentation Abstract (no more than 150 words): Tube exposure is an uncommon but potentially sight-threatening complication of glaucoma drainage device surgery because of the associated risk of infection and surgical failure. This focused presentation provides a practical overview of strategies to prevent and manage tube exposure in routine clinical practice. Key preventive principles include optimal implant positioning, creation of a long intrascleral tube tunnel, appropriate patch graft selection, and meticulous conjunctival handling to preserve tissue integrity and vascularity. Important patient- and surgery-related risk factors will be briefly reviewed to support preoperative planning and risk stratification. A simplified stepwise management algorithm will then be presented, outlining indications for conjunctival advancement with graft revision, tube repositioning or shortening, and device explantation in the presence of infection. Emphasis will be placed on early detection and timely intervention to reduce recurrence and improve long-term surgical outcomes.

Dr. Jella An is Professor of Ophthalmology and Glaucoma Fellowship Director at the Storm Eye Institute, Medical University of South Carolina, USA. Her clinical expertise includes micro-invasive glaucoma surgery, sustained-release drug delivery, and complex cataract and glaucoma care. She serves as principal investigator for multiple clinical trials and has published extensively in peer-reviewed journals. An internationally invited speaker and committed educator, Dr. An focuses on advancing safer, earlier glaucoma treatment while mentoring the next generation of glaucoma specialists through rigorous clinical training and research mentorship
